What on the form MUST be completed by the appellant and what is not required (or can be answered via objection or N/A)?

Above you can see the BFPE response I got when I objected to every line item requested (even name etc.) - all was objected to.

The BFPE can always have changed their opinion that they noted to me some time ago .. mine was not a revocation case FYI
