Didnt want to start another thread but......

For those of you who do not leave the house without a recorder and regularly record yourselves when OCing I was wondering how do you preface your recordings? For instance for each new recording I state my name, date, time, and the manner in which I am carrying and sometimes the destination. Can you think of anything I might be missing?

How do you preface your recordings?
