Over 20,000 hours on the flight deck and nobody thinks checking airspeed on short final might be a good idea? Kinda like shooting yourself while cleaning your gun cuz you didn't bother to check to see if it was loaded first. I can kinda see how the PF (pilot flying) shooting the approach VFR in a new type might not have scanned the instruments as much as he should have. But what the heck was the PM (pilot monitoring), the trainer doing????
Since there appears to be no unrecoverable equipment failures involved, we have to look at all the possibilities when exploring human factors. Now if dingleberry wants to jump up and say it has to be the cultural issues, it doesn't mean he is a racist, it just means he's an idiot. But I think we have all came to that conclusion after SH anyway so I digress.
There are Human factors here, and they may well be cultural. But I am afraid there's an 800 Lb gorilla in the room that nobody wants to look at.
